We are looking for a Full-Time (1.0FTE) RN, Case Manager to join our Hospice team. We provide a 3-month long preceptor-guided training. ROLE SUMMARY The Hospice Case Manager identifies and coordinates care to support terminally ill patients and their families at home, in skilled nursing facilities or at residential care facilities. On average, the Full-Time Hospice Case Manager’s case load will be 14-17 patients.
