Provides basic medical care for patients experiencing homelessness and enrolled in the Recuperative Care Program (RCP). Serves as the clinical liaison with Cottage Health and shelter staff. Ideal candidate will be collaborative, engaging, motivated and have a passion for vulnerable patient populations. Monitor and provide daily insight on RCP bed occupancy while working to ensure patient compliance. Medical Care & Patient Education • Perform initial assessment, including review of hospital charts, discharge paperwork, and collateral information. • Provide oversight to on-site PATH staff delivering individualized health services provided in the RCP. • Explain discharge paperwork to patients and create and help patients execute plan for medical compliance. • Assist patients with medication management practices. • Coordinate with on-site Cottage Health team to write medical care plans for each patient and serves as liaison with hospital staff. • Update patient charts in medical record upon admittance to program, during stay,and discharge from program (when this becomes available) • Provide basic medical care and education for patients • Maintain relationships with and serve as primary contact for home health agencies • Screen for advance directives; set-up advance directives for patients who do not already have them. • Connect patients to a primary care provider per patient choice • Engage patients in utilizing medical home to avoid inappropriate emergency department use. Valid class"C" California Driver License. Verification of acceptable driving record from the California Department of Motor Vehicles. Pulmonary, Renal and Infectious Disease (PRI) is a 32 bed medical-surgical floor with 30 private rooms. Two of the private rooms can be converted into semi-private rooms in times of higher census. The average daily census on the unit is 30.   The patient population of PRI is diverse. We accept all patients age 17 and older. PRI specializes in caring for general medical patients and those patients with Pulmonary, Renal and Infection control disease processes.   The most frequent diagnoses for our patients are: pneumonia, respiratory infections, asthma, COPD, long-term/home ventilators, tracheostomies, pancreatitis, GI bleeds, end-stage renal disease, diabetes and drug and alcohol withdrawal. In addition, PRI specializes in isolation patient care including patients with MRSA, VRE, C-Diff and TB.   Strong staff and excellent teamwork make a position on this unit desirable. Requires a current California License and BLS certification. ACLS certification is desirable.

Based on the Ten Steps to Successful Breastfeeding, this international award recognizes birth facilities that offer breastfeeding mothers the information, confidence, and skills needed to successfully initiate and continue breastfeeding their babies.  Achievement of the Baby-Friendly designation is not an easy task, as there are only 509 active Baby-Friendly hospitals and birth centers in the United States. The U.S. Guidelines and Evaluation Criteria are based on the Global Criteria for the WHO/UNICEF Baby-Friendly Hospital Initiative (BFHI) and adapted for the U.S. Initiative. Located in the city of Santa Barbara, California, Cottage Children's Medical Center is recognized today as a regional referral center for the medical needs of infants, toddlers and children throughout the tri-county area. It is part of the larger Cottage Hospital, a major not-for-profit teaching hospital and trauma center servicing the south and central coast of California since 1888. The primary focus of the 22 bed Level III, Neonatal Intensive Care Unit is the provision of critical nursing care to the infant in crisis.  The neonatal patient may suffer from a wide variety of medical conditions such as prematurity, respiratory distress, meconium aspiration syndrome, hypoglycemia, hyperbilirubinemia, sepsis and surgical conditions requiring pre and post operative care.  Care ranges from the acute phase, to patient needs or conditions necessitating close observation and interventions.  The NICU utilizes a comprehensive multi-disciplinary team approach to meet the patient needs throughout their hospital stay and we recognize the importance of parents and family as being an important member of the team.  We are currently seeking dynamic, skilled neonatal nurses to join our team.  At least 1 year experience in a Level II or III NICU required.
