Position Summary

Woodruff Manor is seeking a dedicated MDS Coordinator (RN) to join our clinical leadership team. The MDS Coordinator serves as an integral member of the interdisciplinary care team and acts as a liaison between residents, families, and staff. This role is responsible for coordinating all aspects of the Minimum Data Set (MDS) process, ensuring accuracy, regulatory compliance, and timely completion of assessments in accordance with CMS and federal requirements.

The ideal candidate will possess strong assessment skills, attention to detail, and a commitment to delivering high-quality resident care while supporting reimbursement and quality initiatives.

Previous MDS experience in a long-term care setting is strongly preferred.

Key Responsibilities

  • Coordinate and oversee the completion and submission of all MDS assessments in accordance with federal and state regulations.
  • Ensure accurate and timely completion of documentation by all interdisciplinary team members.
  • Maintain current knowledge of MDS regulations, reimbursement guidelines, and CMS requirements.
  • Manage and maintain an up-to-date MDS manual, policies, and procedures.
  • Collaborate with nursing, therapy, social services, dietary, and physician teams to ensure comprehensive resident assessments.
  • Monitor assessment schedules and mandated completion timelines.
  • Participate in Quality Assurance and Performance Improvement (QAPI) activities.
  • Analyze assessment data to support quality outcomes and regulatory compliance.
  • Communicate effectively with residents, families, physicians, and interdisciplinary team members.
  • Perform other duties as assigned.

Minimum Qualifications

Education

  • Registered Nurse (RN) with a current South Carolina nursing license.

Experience

  • 1 to 2 years of healthcare experience.

Licensure

  • Current unrestricted South Carolina Registered Nurse license.

Preferred Qualifications

Education

  • Bachelor of Science in Nursing (BSN).

Experience

  • 1+ year of MDS experience in a long-term care setting.
  • 3 or more years of healthcare experience.
  • 1 to 2 years of CMS Assessment Tool experience.

Certifications

  • MDS Certification preferred.
