Job Description:

The Vascular Anomalies Center (VAC) Nurse Coordinator serves as the clinical and operational coordinator for patients receiving care through the multidisciplinary VAC program. This position facilitates referral management, care coordination, clinic operations, patient and family communication, conference preparation, research support, and ongoing clinical monitoring. The coordinator acts as a central point of contact for patients, families, referring providers, and the VAC care team to ensure timely access, continuity of care, and exceptional patient experience throughout the care continuum.

Scope

Responsible for care management, and other related programs within an assigned Clinical Program, functioning with collaborative implementation and evaluation of Clinical Integration Programs, Care Process Models, Care Management, Utilization Management and Patient Teaching. Responsible for quality functions and providing facilitation of positive outcomes.

Essential Functions

  • Coordinate patient referrals, intake, triage, and follow-up across the multidisciplinary Vascular Anomalies Center.
  • Serve as the primary liaison for patients, families, referring providers, and VAC team members, ensuring timely communication and care coordination.
  • Manage patient, provider, and clinic communications through EPIC, email, voicemail, phone, and other communication channels.
  • Coordinate clinic scheduling, pre-visit preparation, post-visit follow-up, and ongoing care planning.
  • Organize and support multidisciplinary VAC conferences, including preparation of case materials, documentation, and follow-up actions.
  • Maintain patient databases, clinical tracking tools, laboratory monitoring records, and shared program resources.
  • Support patient care activities during VAC and medical management clinics.
  • Assist with patient enrollment, coordination, and tracking for approved research studies and clinical trials.
  • Support program outreach, patient education, and family engagement initiatives.
  • Ensure accurate documentation, regulatory compliance, and adherence to established service standards and workflows.


Minimum Qualifications

  • RNs hired or promoted into this role need to have or obtain their BSN within three years of hire or promotion.
  • Current RN Licensure in the State of Utah.
  • - and -
  • Basic Life Support (BLS) Certification
  • Three years clinical hospital experience.
  • - and -
  • Exceptional oral and written communication skills.

  • Strong ability for conceptual thinking, problem resolution and planning skills.

Preferred Qualifications

  • Bachelors degree in Nursing (BSN) from an accredited institution (education will be verified).
  • Pediatric nursing experience.
  • Experience in care coordination, patient navigation, case management, or nurse navigator roles.
  • Experience supporting multidisciplinary specialty programs.
  • Experience with clinical research coordination or patient enrollment activities.
  • Familiarity with vascular anomalies, hematology/oncology, genetics, dermatology, interventional radiology, or other related pediatric specialties.
  • Experience utilizing EPIC and shared database management tools.
  • Certified Pediatric Nurse (CPN) or other relevant specialty certification.

Ongoing need for employee to see and read information, labels, monitors, identify equipment and supplies, and be able to assess patient needs.

Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, etc.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$45.31 - $67.01
