Summary
Summary
Supports the department Manager and Director in the daily operations of the department; and in mentoring and professional development of staff. Responsible and accountable for translating the mission, strategic goals and department priorities. Supports and maintains a culture of continuous process improvement as related to care coordination and case management. Works collaboratively with the health care team to remove barriers and achieve the desired outcomes for patient/families. Develops informational and educational content for department meetings as required. Supports manager in leadership and development opportunities and provides performance input to the manager for staff yearly appraisals.
Education
Bachelor of Science in Nursing (BSN) required.
Licensure/Certification
Must be an RN with current licensure. CPHQ or CCM highly desirable.
Experience
Preferred recent experience in acute care setting. Preferred experience in Case Management. Preferred experience or knowledge of InterQual Criteria.
Other Qualifications
Must have a broad base of clinical knowledge and understanding of pathology and physiology. Must have good verbal and written communication skills. Must be available to work flexible hours, assuming on-call duties on weekends and other times when deemed necessary. Must be available to respond within a reasonable length of time. Possesses strong critical thinking skills. Ability to lead and direct co-workers in a positive and professional manner. Demonstrates continued professional development. Strong working knowledge of information technology. Ability to work collaboratively to achieve desired outcomes.
