In 2020, our program distinguished itself as thethird in the nationto achieveHeart Failure Certification through DNV, reflecting commitment to evidence-based standards and a culture of continuous improvement. In 2022, we furtherdemonstratedour commitment to exceptional outcomes by achievingGold Plus Status from the American Heart Association, a designation we continue tomaintainthrough sustained performance in quality metrics and guideline-directed care.

Our multidisciplinary team plays a critical role in advancing patient outcomes through comprehensive education, individualized transition planning, and long-term disease management.
