Registered Nurse (RN)-Case Manager

The Health Resource Manager is responsible for providing case management services for SVMC patients.

Requirements:

  • Graduation from an accredited school of nursing
  • Prior experience in utilization review, discharge planning and/or case management preferred but not required.
  • 2 years of recent clinical experience required.
  • A Vermont RN Nursing License is required by the date of employment.
  • Current BLS Certificate

The case manager is responsible for coordinating a patient-centered plan of care to meet individual needs by:

  • Completing intake interviews to identify a holistic plan of care; facilitating multiple aspects of care coordination with the patient, family/caregiver, providers and other community resources prior to discharge
  • Developing a patient transition plan, providing patient/caregiver education and proactively advocating for patient needs and goals across the interdisciplinary team to ensure an approach that is sensitive to each specific patient and family’s needs
  • Assisting providers in determining patient’s level of care in accordance with InterQual criteria
  • Actively seeking out collaboration with providers to improve overall quality and efficiency of care
