As part of our mobile clinic unit program, you will play a pivotal role in providing vital primary care services and fostering community health and wellness.

Responsibilities:
• Deliver high-quality healthcare services directly to patients. This includes performing health assessments, administering treatments, and providing patient education to promote health literacy and self-care. Some care coordination may be needed.
• Engage with community members to build trust, rapport, and awareness of available healthcare services. Foster partnerships with local organizations, leaders, and stakeholders to maximize outreach and impact within target communities.
• Encourage health promotion and disease prevention initiatives to empower individuals and communities to adopt healthy behaviors and lifestyles. Provide educational materials on topics such as nutrition, hygiene, chronic disease management, and preventive screenings.

Capable clinician, focused on expanding knowledge and skills. Consistently provides effective direct care, as part of the interdisciplinary team, to a variety of complex patients. Manages care and implements treatment plans at a refined skill level in collaboration with patients, their families, physicians, and other members of the health care team. Seeks as well as provides feedback for improved clinical practice. Assumes a beginning leadership role but seeks mentoring in this process.

MINIMUM REQUIREMENTS

Education: Graduate of an accredited nursing program required. Bachelor of Science in Nursing required within 5 years of hire.

Experience: 1 year of relevant experience.

Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Healthcare Provider BLS certification required.

Preferred Requirements:

  • 2 years of nursing experience. Experience in community health nursing, primary care, or public health is a plus!
  • Strong communication, interpersonal, and organizational skills.
  • Ability to work independently and collaboratively 
  • Commitment to health equity, social justice, and serving vulnerable populations.
  • Willingness to travel to various locations within the community served by the mobile clinic van.

PHYSICAL DEMANDS

Job requires standing for prolonged periods, frequently traveling, bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids and infectious disease

The starting base rate for this role is $37.51 hourly. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
