Loan forgiveness and tuition reimbursement programs are available.   RESPONSIBILITIES Conduct comprehensive discharge planning assessments from admission through hospitalization Identify and resolve barriers to discharge, including medical, psychosocial, and financial factors Develop and coordinate individualized discharge plans with patients, families, and interdisciplinary teams Arrange post-acute care services such as home health, skilled nursing, hospice, and community resources Provide patient and family education on care plans, medications, and follow-up using teach-back KNOWLEDGE AND EXPERIENCE Active Registered Nurse (RN) license in the State of Alaska Bachelor of Science in Nursing (BSN) with at least 2 years of acute care experience Strong clinical judgment, communication, and care coordination skills Experience with discharge planning, case management, or care transitions preferred Ability to manage multiple priorities in a fast-paced healthcare environment Proficiency with electronic medical records (EMR) systems Knowledge of payer requirements including Medicare, Medicaid, and private insurance Physical Requirement: Ability to stand, walk, sit for extended periods, and occasionally lift up to 25 pounds ENVIRONMENT The South Peninsula Hospital is a combined 22-bed Acute Care Critical Access Hospital and 28-bed Long Term Care facility located at the southern end of the Kenai Peninsula in South-Central Alaska. This role is based in a fast-paced, patient-centered hospital environment where collaboration and communication are essential to success. The position requires balancing multiple priorities while maintaining attention to detail and delivering compassionate care. Work schedules may vary depending on operational needs.
