Summary of Primary Function/General Purpose of Position

This position is responsible for the development and implementation of Quality and Process Improvement initiatives across all Community Health Access programs including but not limited to the Women’s Health Services, Mobile Health Services, Preventative Health Services, and Community Benefit Implementation. The position is a member of the community health access leadership team and provides oversight, guidance, and direction to leaders and program staff on strategic quality and process improvement initiatives.  Work is performed under the general direction of the Director, Community Health Impact Team.

               

Essential Job Functions

  • Responsible for developing, monitoring, and evaluating comprehensive, clinical services across all community health access programs

  • Serves as a liaison between community health programs and internal regulatory and BSMG leadership

  • Develops and revises clinical guidelines and procedures for clinical teams and ensures implementation and accountability

  • Develops and leads innovative clinical and process workflows to improve quality of care and patient experience

  • Works with leaders to interview, hires, and orients new employees; teaches, educates, provides guidance on best practices

  • Ensure all community health programs comply with applicable best practice standards of care, legal guidelines, and safety guidelines.

  • Ensures all employee files are up-to-date with appropriate licensures

  • Creates Standard Operating Procedures, Workflows, and other staff and patient facing documents 

  • Monitors and evaluates quality of patient care through detailed quality dashboards and reports monthly to leadership team

  • Periodically reviews provider/nursing/medical assistant’s documentation in patient EMR for compliance

  • Acts as liaison between community health programs and students seeking placement opportunities including the Bon Secours College or Nursing and St. Francis Residency

  • Arranges for continued educational programs for clinical and non-clinical staff

Required Minimum Education:

4 year/ Bachelors Degree

Specialty/Major:

Nursing, Quality, Patient Experience, Public Health, Data Analytics

Preferred Education:

Graduate Degree (Masters)

Specialty/Major:

Licensure/Certification Required:

Current licensure in the State of Virginia as a Registered Nurse

Licensure/Certification Preferred:

Public Health Nursing, Lean Six Sigma

Minimum Years and Type of Experience:

7-10 years of experience in nursing, patient care, or healthcare quality

Other Knowledge, Skills and Abilities Required:

Knowledge of the principles, practices, and techniques of professional public health nursing. Considerable knowledge of physical assessment, pathophysiology, preventive health, and health education as well as acute and chronic health conditions. Ability to prepare reports and to communicate effectively both orally and in writing. Ability to work independently as well as a member of a team. Ability to work independently as well as a member of a team. Ability to gather and analyze data.  Understand basic statistics. Requires comfort level with computer office and business applications such as Microsoft Office, Excel, and PowerPoint. CPR certification required
