Job Description Summary The successful applicant will be an Advanced Practice Provider interested in working full -time in the care of patients requiring Palliative Care in our outpatient Adult Palliative Care clinics. The APP will join a busy a nd thriving Adult Palliative Care team that includes 7 Faculty, 2 Fellows, 4 inpatient APP’s, 3 Palliative Telehealth APP’s, and 2 Adult Palliative outpatient APP in a growing practice. Outpatient Palliative Care services include symptom management, care coordination with the patient’s other medical providers, ongoing discussions regarding treatment choices, and quality of life goals.
