A registered nurse is responsible for managing and coordinating the plan of care for hospice patients in their place of residence and to coordinate care with the Interdisciplinary Team, patients, families, referring agency/facility, and/or caregivers. The RN administers nursing care for patients utilizing all aspects of the medical plan and nursing standards of care. Directs and guides patient/family education and the activities of other personnel while maintaining standards of professional nursing. Must possess the ability to assess data reflective of the patient’s status and interpret the information to formulate a plan of care based on the patient’s age-specific needs. Team members working on this unit are early adopters of new technology, new equipment, and nursing practice models. Nurses on this unit care for adult patients who are acutely ill with a wide variety of medical problems. The patient population on Medical Innovation includes renal, pulmonary, infectious disease, GI issues, hospice, and more. The service will initially focus on Cardiology inpatients, with an anticipated expansion to Hospital Medicine, other inpatient populations, and, ultimately, into ambulatory settings. The incumbent APP will work closely with the Health System's Director for Advanced Care Planning on programmatic development and growth, affiliated physician staff, and divisional APP leadership related to operational role development. The APP will provide clinical and operational leadership which will include expanding the service to include additional ACP consultants as necessary.
