Description The Structural Heart Nurse Navigator functions on the multidisciplinary team as a clinical expert, patient advocate, educator, coordinator, and liaison for all structural heart patients and their respective family members. Responsible for ensuring all patients navigating within the structural heart program receive quality and comprehensive services. Coordinates patient care throughout the continuum in collaboration with the multidisciplinary team.
