32765000 Description Summary: Under the supervision of the Director of Cancer Center, the Registered Nurse Patient Navigator Senior is responsible to ensure interdisciplinary, patient-focused, well-coordinated system of care for oncology patients coping with Cancer. Collaboratively assess, plan, facilitate and evaluate timely coordination of quality care for the cancer patient. Functions on the multidisciplinary team as an advocate and educator for oncology patients. Responsible for ensuring all adult patients with an oncology diagnosis receive quality and comprehensive services. This role coordinates patient care throughout the entire continuum of cancer care, in collaboration with the multidisciplinary team.
