Initiates and completes patient assessments throughout the continuum of care (admission to discharge); identify physical, social and psychological response of patient and family using interviewing and physical assessment skills. Modify standard of nursing care to reflect needs and changes in patient status; sets reasonable goals and designates intervention to treat identified problems/needs.
