Job Description Summary Responsible for planning, coordinating, triaging, and documenting care for home health and/or hospice patients during hours when the office is closed. Utilizing physicians' orders and professional skills the RN develops, follows, and implements a plan of care that meets each patient's specific needs. Evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcomes. The On Call RN is responsible for triaging patient care issues, and following up with a visit if needed. The On Call RN facilities patient admissions and discharges as needed.
