Job Description: Position Summary Under the leadership of the director and assistant director of Hospice of the Central Coast, the Case Manager is a Registered Nurse responsible for the smooth operations of the hospice office, fielding informational calls that require an RN, speaking with MD’s, taking telephone orders, assisting nurses in the field, speaking and instructing families and patients on hospice. The Hospice Case Manager possesses the skills and knowledge to competently provide appropriate nursing care on the phone or in person.
