Description Advanced Practice Provider Advanced Care at Home (ACH) Program – UNC Health About the Program UNC Health’s Advanced Care at Home (ACH) program is redefining inpatient care by safely delivering hospital-level services in a patient’s home. Using a combination of virtual care, in-home clinical visits, and a centralized Command Center, ACH achieves clinical outcomes equal to or better than traditional hospitalization - while significantly improving the patient experience. Patients receive access to many inpatient services, including medication management, mobile imaging, home infusion, lab services, rehabilitation, nutrition consults, and more -all without leaving home. Care is fully integrated with primary care providers, specialists, and ancillary services across UNC Health.
