UOVUNIUSR0080550EXTERNALENUS The Advanced Practice Provider (APP) will launch and lead a dedicated Advance Care Planning (ACP) consultation service within UVA Medical Center and affiliated with UVA School of Medicine's Division of Hospital Medicine. The service will initially focus on Cardiology inpatients, with an anticipated expansion to Hospital Medicine, other inpatient populations, and, ultimately, into ambulatory settings. The incumbent APP will work closely with the Health System's Director for Advanced Care Planning on programmatic development and growth, affiliated physician staff, and divisional APP leadership related to operational role development. The APP will provide clinical and operational leadership which will include expanding the service to include additional ACP consultants as necessary. The Nurse Practitioner will be responsible for providing clinical care, support, and education for the palliative patient population as part of the health care team.
