Description
Summary:
Collaborates with the nursing and provider care teams to deliver effective, timely admission and discharge of patients. Meets the physical, psychosocial, religious/cultural and needs of the patient and family. Communicates effectively with all members of the health care team as appropriate. Flexible, organized and functions under stressful situations.
Responsibilities:
1. Performs duties as assigned and requested.
2. Monitors admissions for anticipated length of stay and discharge date.
3. Identifies anticipated needs for teaching, caregiver support, and details regarding discharge prescriptions and/or transportation for discharge.
4. Serves as a liaison for facility regarding discharge.
5. Ensures all discharge related paperwork has been initiated.
6. Performs patient care as needed and communicates effectively with patients of all ages and specific needs. Ensures contact with case manager, social worker, or others to identify needs requiring intervention.
7. Monitors discharge logs and assists with reporting of outcomes to meet goals.
8. Demonstrates knowledge of medications and their correct administration based on the age of the patient and the patient’s clinical condition.
9. Assists physicians with procedures and performs services requiring technical and manual skills.
10. Performs treatments and provides services to the level of licensure.
11. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of errors.
12. Manages and operates equipment safely and correctly.
13. Assists case managers with discharge plan(s)

Education
Bachelor’s degree is preferred
Licensure/Certification
Registered Nurse in North Carolina is required (or compact state)
Experience
Minimum 3 years of applied clinical experience as a Registered Nurse.
Minimum 1 year hospital case management experience preferred.
Knowledge, Skills and Abilities
Valid NC Driver’s License: No
If driving a Wayne UNC Vehicle, must be 21 years old and MVR must be approved by Risk Management.
