Job Overview: RWJBarnabas Health is seeking a highly dedicated Wound Care Coordinator RN to work in the Develop Innovation & Research department at Monmouth Medical Center Southern Campus. Monmouth Medical Center Southern Campus is a fully accredited acute care hospital dedicated to providing the finest medical and health care services to the residents of Ocean and Monmouth counties. An affiliate of the RWJBarnabas Health system, Monmouth South is a sister hospital to Monmouth Medical Center in Long Branch. Monmouth South is the only hospital in the region to provide private rooms on all of its inpatient units, providing an environment more conducive to healing. Qualifications: Required: ASN or Nursing Diploma Strong communication and organizational skills Proficient computer skills Certifications and Licenses Required: BLS and ACLS AHA certification Active New Jersey Registered Nurse License Preferred: BSN Minimum of 1-year previous Wound Care experience Wound certification or completion within 1 year of hire Ostomy certification Scheduling Requirements: Day shift, Per Diem   Essential Functions: The Wound Care Coordinator performs comprehensive wound assessment and reassessment in collaboration with the patient s primary care team The Wound Care Coordinator evaluates appropriateness of wound consults and follow-up The Wound Care Coordinator develops and implement individualized treatment plans for all patients throughout the hospital related to wound prevention and wound management per policy and within scope of practice The Wound Care Coordinator acts as a liaison with physicians and other health care team members and helps to coordinate patient care The Wound Care Coordinator provides/formulates patient/family education based upon the individual needs of the patient The Wound Care Coordinator performs thorough documentation, including pictures, wound assessment/ staging, wound reassessment and treatment plans The Wound Care Coordinator acts as a wound management, wound documentation and wound prevention resource for nursing staff and providers The Wound Care Coordinator participates in house-wide skin/wound care programs for staff education and patient care as needed including Prevalence and Competency Days The Wound Care Coordinator develops and participates in unit and hospital-wide performance improvement plans, including wound prevention and continence care The Wound Care Coordinator uses an understanding of the principles of growth and development to assess each patient s age specific needs and provide age specific treatment and care Choosing RWJBarnabas Health!
