As the LPN Nurse Coordinator at Westhaven, you will provide hands-on leadership for a combined Transitional Care Unit (TCU) and Long-Term Care (LTC) unit, supporting nursing staff while ensuring exceptional resident care. You’ll collaborate closely with the Director of Nursing, assist with admissions, oversee wound care, and help maintain regulatory compliance. LPN Nurse Coordinator Responsibilities:
Work to coordinate, oversee, and provide resident-related care.
Practice restorative nursing to provide the best care to our residents.
Coordinate and assign tasks to nursing staff on the assigned unit, which includes both Transitional Care (TCU) and Long-Term Care (LTC).
Work with the DON to ensure compliance with state and federal regulations.
Notify Clinical Manager, attending physicians, and family members of significant changes in a resident's condition, and take follow up action as necessary.
Oversee wound care management, including treatment and documentation.
Monitor in-house transfers, transfers to other facilities, and discharge planning.
Document and maintain paperwork, referral records, and reports.
Display a courteous attitude and respect for all residents, families, and staff.
Step in to support the nursing floor and provide direct patient care when needed.
Participate in an on-call rotation to support clinical operations.
Performs other duties as assigned.
LPN Nurse Coordinator Qualifications:
Must have a current Iowa LPN license and be in good standing.
Must have CPR certification or willing to obtain.
LTC or TCU experience preferred.
Proficient in MS Word, Excel, and MS Outlook.
Strong communication and customer service skills.
