Job Description:
The RN Clinical Liaison reports directly to the director and assistant director of Hospice of the Central Coast and is responsible for delivering appropriate services to patient populations of her/his team and as assigned during scheduled working hours. The RN Clinical Liaison possesses a broad knowledge and skill level to competently provide patient education in community resources, crisis intervention, advance care planning, and hospice evaluation. The RN Clinical Liaison functions as a core member of the IDG team. The RN Clinical Liaison will provide outreach to the facility partners and the community at large and will assist with increasing census by promoting and facilitating referral services. The RN Clinical Liaison’s primary role is outreach to the community.
Experience
One year experience as a nurse is required. A minimum of two years home health experience within the last five years is preferred. Must have experience working independently, strong interpersonal skills, excellent customer service skills, and strong computer skills. Experience in community relations and marketing highly preferred. Spanish speaking required.
Education
Bachelor’s degree in nursing preferred.
Must pass orientation and initial competency assessment prior to independent assignment of patients requiring the applicable skill, and must complete and pass all annual competencies.
Licensure/Certifications
State of California RN license required. American Heart Association Healthcare Provider BLS required. Hospice certification preferred. Must possess a valid California driver’s license.
This position requires driving. In addition to a valid CA driver’s license, the following are required: Automobile insurance as required by the State of California with an acceptable driving record of three points or less, participation in the DMV pull-notice program, and an automobile available for daily work.
