Job Overview:
Asthma Program Clinical Manager is responsible for managing the implementation and operations of the asthma program at CHoNJ, to include oversight of asthma initiatives at multiple sites as programmatic expansion occurs. Responsibilities include communication with internal and external stakeholders, coordinating reports and statistics, analyzing performance, and exploring additional opportunities for programmatic growth. Provides specific asthma education to patients and families, both directly and indirectly.This position works closely with the physician lead and a multidisciplinary team that focuses on the goal to achieve optimal asthma control and decrease the risk of adverse events. In addition, the asthma education coordinator monitors key metrics and analyzes information to ensure operational quality and efficiency targets are being met.
Qualifications
Required:
Associates Degree in Nursing
Minimum of two years-experience in leading an asthma program, preferably in a healthcare setting
Strong understanding of treatment protocols and guidelines, including knowledge of new and emerging treatments
Proven track record in developing and implementing successful asthma education and outreach initiatives,
Excellent communication skills
Ability to work effectively with healthcare professionals, patients, and community partners
Preferred:
Bachelors Degree in Nursing
Certifications and Licenses Required:
NJ RN required
Essential Job Functions:
Program Leadership
In conjunction with lead physician and departmental leadership, works to ensure the asthma program is based on current evidence-based practice/guidelines
Oversees collection and utilization of operational and benchmarking data to recommend targets for improvements
Provides guidance to others involved in the program, including expansion to additional RWJBH sites
Evaluates and reports program status and changes to physician and/or organizational leadership
Oversees strategic and operational program projects
Drives outcomes defined by program deliverables
Service Delivery
Participates in, provides, and arranges program education, ensuring Asthma Action Plan is correctly completed with information specific to the patient
Collaborate with patient, family, and interdisciplinary care team to establish goals for self-management skills training and education and ensure patient and caregivers have all needed resources at time of discharge
Educate and mentor other disciplines around the impact of health literacy on patient care and outcomes
Provide initial and continuing education and skills training for children and caregivers on the care that will be required upon discharge from the hospital and across the care continuum
Role models service excellence in interactions with internal and external constituents
Establishes strong cooperative relationships with applicable leaders by providing the information, tools, and services necessary to carry out their leadership responsibilities
Communication
Designs and orchestrates the delivery of applicable communication to medical staff, other healthcare team members, and leaders to establish goals and strategies related to the program
Coordinates presentations and ensures consistent communication related to program assignments
Operational Improvement
Recommends and participates in the review and implementation of program process improvements, monitoring and measuring process changes made
Facilitates cross-functional performance improvement teams
Research best practices and recommends process improvements to leadership based on the research
Reviews and recommends policies that support the program
Community Engagement
​​​​​​​Working in collaboration with RJWBH community outreach team, assist in the implementation of effective outreach programs relating to asthma prevention and education including local school system administrators, health fairs, etc.
