This role ensures high‑quality clinical care through thorough assessments, timely interventions, and compassionate support. The nurse collaborates closely with the home health multidisciplinary team, following physician orders and working under the direction and supervision of the Clinical Supervisor. Responsibilities Provide skilled nursing services to home care clients in accordance with individualized care plans, physician orders, and regulatory guidelines. Perform comprehensive patient assessments, including physical, psychosocial, and environmental evaluations. Administer medications, treatments, wound care, and other clinical procedures as appropriate for the patient’s needs. Educate patients and caregivers on disease processes, treatment plans, medications, and home safety. Communicate changes in patient condition to the physician and clinical team promptly. Collaborate with other members of the multidisciplinary team Maintain accurate, timely, and compliant documentation in the electronic medical record Adhere to infection control standards and ensure patient safety in all care settings. Provide compassionate, patient‑centered care while promoting independence and optimal health outcomes. Qualifications Minimum of one (1) years recent experience as a professional nurse in a clinical setting. Home Health experience preferred. Must complete Point of Care Testing training as part of Department Orientation. Valid DL With No More Than 3 Moving Violations Proof Of Current/Valid Car Insurance Competencies Skills Ability to operate safely and confidently in various home environments Strong clinical assessment and critical‑thinking skills. Excellent communication, documentation, and patient‑education skills. Ability to work independently in a home setting while collaborating effectively with the care team. Able To Keep Confidential Information Regarding Patients, Team Members Has Skills To Provides Customer Service To Patients, Team Members And Visitors Knowledge And Experience With Electronic Health Records   Credentials RN - Registered Nurse BLS - Basic Life Saving certification (American Heart Association) Entry USD $36.20/Hr.
