Summary:
The Family Nurse Practitioner (FNP) manages the WATCH Healthcare Program for Wayne County Citizens. Under the supervision of the physician, delivers acute, basic and preventative health care to clients and fulfils duties such as assessment, planning, teaching and counseling. Functions as a health team member in meeting the objectives of WATCH.
WATCH stands for Wayne Action Teams for Community Health. The mission of WATCH is to provide quality primary and acute health care to the uninsured of Wayne County, North Carolina.
Responsibilities:
1. Patient Care:
a. Obtains health and development history and conducts physical assessments using appropriate tools. Conducts gynecological exams i.e., GC, papsmear, STD screenings, etc.
b. Assesses the health status of the client by discriminating between normal and abnormal physical and emotional findings.
c. Orders appropriate screening and/or routine diagnostic tests.
d. Records findings in a systematic and accurate form.
e. Provides treatment for minor illnesses and accidents either in consultation with the physician or according to established protocol.
2. Drives and operates the mobile unit safely.
3. Conducts telehealth as indicated.
4. Patient Case Management:
a. Participates in and, when appropriate, continues health care plans involving client, family or other community agencies and resources.
b. Evaluates data from assessment and ascertains client needs for family planning, substance abuse and/or other health care needs.
c. Makes pregnancy determination as indicated and maintains continuity of client care for pregnant clients by referral to the appropriate agency. Counsels regarding sexual issues including family planning.
d. Identifies resources available in the community to help clients and their families, and assists families in accessing necessary resources.
e. Encourages families to call with concerns and provides appropriate phone management of these concerns.
f. Follows up on emergency hospital visits as appropriate.
g. Follows up on missed appointments or continuity of care as appropriate.
5. Mobile Unit and Healthcare Operations:
a. Contributes to healthcare program planning, evaluation, report preparation and development of research.
b. Supervises ordering and purchasing of lab and medical supplies.
6. Maintains Professional Responsibility in Program and Community.
7. Contributes to health education of individuals and groups when needed.
8. Keeps Medical Director, Executive Director and Clinical Manager informed and up to date regarding issues, concerns or problems.
9. Identifies learning needs through self-assessment and established goals to demonstrate professional growth and development. Participates in continuing education and in-services. Shares information appropriately with WATCH program teammates.WAYNE
Other information:
Education
Graduate of an accredited School of Nursing with MSN Degree.
Licensure/ Certification
Current registration to practice in North Carolina as a Nurse Practitioner required.
Current Registered Nurse license to practice in North Carolina required.
National certification as a Family Nurse Practitioner required.
PA-Fifth year college or university program certificate or two to four years related experience or training, or equivalent combination of education and experience.
Certificate of valid registration to practice in North Carolina.
Experience
Prefer 2-3 years of experience working in a family practice office or health clinic setting.
Supervisory experience required.
Knowledge, Skills and Abilities
• Bilingual a plus.
• Excellent interpersonal and customer service skills. Ability to build rapport with diverse patients.
• Leadership qualities, e.g., maturity, job knowledge, professionalism, teamwork and communication skills, etc. to lead, guide and motivate others.
• Outstanding problem-solving skills.
• Ability to read, write and communicate effectively in English.
• Proficient with MS Office and Epic EMR, and able to learn new software rapidly.
Valid NC Driver’s License: Yes
If driving a Wayne UNC Vehicle, must be 21 years old and MVR must be approved by Risk Management.
