Prescribes medications, diagnostic testing and manages plan of patient care.   Responsibilities 1) Provide patient care via evaluation and treatment of patient conditions. 2) Prescribes medications as needed for patients. 3) Prescribes therapy or diagnostic testing as needed. 4) Educates patients on health care concerns as needed. 5) Coordinates care as needed in the patient centered medical home model of care.
