The Orchard is seeking a Full Time MDS Coordinator / Admissions Nurse to join our team! The MDS/Admissions Nurse works under the guidance of the Facility Administrator in partnership with all transition team members and household staff. The MDS/Admissions Nurse promotes rehabilitation guests' wellbeing and satisfaction by supporting person-centered and guest-directed care. Participates in assessing, planning, implementing and evaluating nursing care and provides for the emotional and physical comfort and safety of rehabilitation guests and their families. This individual is a role model for professional nursing practices. As an employee of the household, the MDS/Admissions Nurse is considered a versatile worker and is cross-trained in different functions of the household within scope of licensure (if applicable) in order to meet guest needs as they occur. In addition, this team member oversees the entire RAI process focusing on delivery of care resident outcomes reimbursement and quality assurance of the RAI process. Ensures accurate and timely completion of all areas of the assessment CAA completion and Care Plan development and resident outcomes. Qualifications Education: Graduate from a Professional Nursing (RN) program required. May consider candidates that are actively pursuing an RN degree. Experience: 2 Years long term care experience preferred. 1 year experience with the RAI process preferred. Licensure/Certification: Current unencumbered license as a Registered Nurse or a mult-state licensure privilege to practice in Virginia. May consider candidates with an active LPN license in good standing if they are actively pursuing an RN degree and licensure. MDS certification highly preferred.
