As a Clinical Quality Improvement Practitioner you need to know how to: Work collaboratively with physicians, other clinicians, multidisciplinary PI teams, and IT to implement evidence-based practice. Conduct literature review and synthesizes evidence-based practice on a broad range of topics; makes sound recommendations to the relevant leaders of system collaborative and improvement initiatives in key patient cohorts or patient safety indicators as defined by the organization’s goals and priorities including CMS Five-star attainment. Develop content for change packet for various acute care initiatives – using preexisting content or developing new content where appropriate to drive the education of clinicians (physicians and staff) in relevant delivery area and service lines. Considers the inpatient as well as post discharge evidence-based practices to facilitate reductions in 90-day morbidity and mortality. Will have a thorough understanding of the value proposition (Quality/Cost) across the continuum of care. Is well versed in the metrics of governmental Pay for Performance programs, including bundled payment. Will provide expertise in development of, and review of, clinical policies & procedures in conjunction with clinical leaders. May be required to travel to care sites, approximately 10%. But we also expect excellence, which is why we require: Master’s Degree in Nursing (MSN) from a nationally accredited graduate degree program or a nationally accredited postgraduate certificate program, required Board certified Advanced Practice Nurse (APN), required Unrestricted APN license by State Board of Nursing (Colorado or other state board of nursing. Eligible for Colorado licensure within 3-6 months of hire), required Minimum of three (3) years’ experience using APN degree in the acute care setting, required Clinical Nurse Specialist (CNS), RN preferred Your next move.
