Overview The Registered Nurse, Case Manager is a Bachelors required role with NH Licensure as a RN. The Case Manager works to assess patients upon admission and to ensure a safe and timely discharge for patients with all the education and resources in place for their discharge. The Case Manager works collaboratively with providers, residents, RN's and other multi-disciplinary staff.
