Position reports to FHH Associate Director and works directly with Clinical Services Manager (field) and Hospice House Assistant Director to assist with and coordinate daily duties in a fast paced hospice office setting to efficiently meet patients/caregiver needs.
Responsibilities include triaging hospice RNs calls from the field, taking physician orders, coordinating pharmacy & DME needs, assisting with patient referral process. Will also participate in committees to improve processes and assists with scheduling. Assists with orientation of new staff, reports/records/documents information accurately, demonstrates knowledge of Medicare, Medicaid, and insurance regulations pertaining to hospice, participates in clinical record review for quality and compliance, maintains and submits logs, schedules and reports within required time frames.
Two years of home care or hospice related experience, med-surg or acute care experience required. Must have ability to communicate effectively with all levels of the work force- clinical and non-clinical. Must have understanding of hospice philosophy and knowledge of CMS regulations, compliance, documentation standards and HIPAA. Must have demonstrated computer skills, be team oriented and have good organizational skills with the ability to manage multiple tasks and meet deadlines.
