Job Description:
The Nurse Practitioner (NP) is an expert in diabetes care. She/he is responsible for the delivery of diabetes care and education that involves compliance with evidence-based clinical standards, outcomes measurement (tracking, analysis, and reporting of clinical, financial, and satisfaction outcomes) and process design (leading initiatives to facilitate practice-based and organizational change). As a member of the glycemic management team, the NP provides direct patient care in the inpatient, outpatient and community setting, through implementation of standardized procedures and works collaboratively with the medical team to establish priorities and to assess solutions for patients to ensure they receive the best care possible.
The NP functions under general Nurse Practitioner Standardized Procedures approved by the Interdisciplinary Practice Committee (CIDP) and reports to the Diabetes Services Manager.
Experience:
Clinical background in diabetes management is required. Minimum of 2 years acute care hospital experience is preferred. Experience in managing insulin pumps is preferred and experience in outpatient diabetes services preferred. Spanish fluency preferred.
Education:
Master’s Degree in Nursing and relevant Nurse Practitioner training/certification.
Licensure/Certification:
Current California RN licensure and authorization as a nurse practitioner required. Certified by a recognized national certifying body that has established standards for nurse practitioners. Furnishing license issued by the Board of Registered Nursing required. Certified Diabetes Care and Education Specialist (CDCES) credential must be completed within 1 year from hire date or at the completion of 1000 hours of providing patient diabetes education. Board Certified-Advanced Diabetes Management (BC-ADM) must be completed within 2 years from date of hire. If hired before 7/31/2020, (BC-ADM) must be completed within 2 years from 7/31/2020. American Heart Association Healthcare Provider BLS certification required.
