Job Description
Summary
Responsible for assuring the delivery of quality nursing care for a designed school and/or group of students/parents/staff in a specified community setting. Responsible for the growth and development of outreach programs in the community that will foster improved health and wellness.
We have 3 roles we are looking to fill and have the flexibility to find applicants that may fulfill one of our needs depending on the skills sets and experiences of nurses applying for this newly posted position:
- Homeless programming: Seeking a nurse with experience navigating clients with chronic diseases and mental health issues, working with clients experiencing mental illness, substance use disorder, and homelessness,. This nurse will work in a clinic setting at community homeless shelters navigating care and connecting clients to needed community resources.
- Family and Children programming: Seeking a nurse to help co-lead breastfeeding support groups and help grow this program in the community, CLC or IBCLC certification desirable, minimally has experience working with breastfeeding moms if not certified. This nurse will also be working with our community partner agencies, assisting agency staff and families with children with health needs, helping navigate care, providing education, and connecting to community resources as needed.
- School programming: Seeking a nurse with experience working with school-aged children and parents/families. This nurse will be working in a clinic setting administering immunizations, completing assessments and screenings, navigating families to community resources. This nurse will help teach programs to middle school-age students.
Education
Must obtain Bachelor of Science in Nursing (BSN) within three years of employment or will not be eligible to continue in the role. Coursework is completed outside of work hours. Nurses with an Associate of Science in Nursing (ASN), hired/rehired or transferred from a non-nursing position prior to 12/1/17 are grandfathered into the role and not required to obtain BSN.
Licensure/Certification
CPR instructor required; or must obtain within one year of employment. Certification in Diabetes, Asthma, Pediatrics, or School Nursing preferred (Certification may be required dependent on job assignment.). Valid IN Registered Nurse required. Must have a valid driver's license. Must provide current proof of auto insurance.
Experience
Must have 3 to 5 years of RN experience, with preferable outreach, project management or education background.
Other Qualifications
Familiarity with designated service area and surrounding communities as it relates to outreach and health initiatives.
