Job Summary
Facilitates the coordination of the patient care activities in collaboration with the multi-disciplinary team for the sepsis population. Coordination efforts are across the continuum from the emergency department, critical care units and inpatient areas of all Prisma Health hospitals. Assists in the development of database to track and quantify cost, quality and outcome measures for this population; provides data extraction and analysis for reports.
Accountabilities
* Actively participate in quality improvement activities. Reviews EMR for real time extraction of process analysis and identification of improvement opportunities. Identifies, collects, enters, monitors and evaluates indicator that reflect the important aspects of care and service. Develops and collaborates with other members of the healthcare team to implement changes to improve outcomes. 30%
* Serves as a clinical expert and resource, assisting in the care for complex and/or highly specialized patient care situations. 25%
* Coordinates the multidisciplinary committee from across the continuum; makes recommendations for process improvements with clinical processes for all Prisma Health facilities. 15%
* Analyzes data and prepares reports; assists leadership in setting goals, identifying roadblocks, and in establishing milestones for project success.10%
* Responds when appropriate to Code Sepsis calls from the ETC and other areas of GMH for real time quality improvement observations and interventions. 10%
* Initiates and participates in case reviews for Sepsis patients. 5%
* Develops this new professional role. Participates in Prisma Health committees as appropriate. Remains current in literature and best practice. 5%

This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Education
Bachelor's Degree in Nursing
Minimum Experience
3 years- Nursing, experience in intensive care or ED Nursing preferred.
Required Certifications/Registrations/Licenses
RN - REGISTERED NURSE - NLRN
Other Required Experience
Experience working with Excel, databases- Preferred
Experience working with patients with a diagnosis of sepsis in an ICU setting or ED setting- Preferred
