Job Summary: Provides medically prescribed skilled nursing services in the home setting using the nursing process. Implements nursing actions that will promote, maintain, or restore physical health, prevent illness, and effect rehabilitation.

Minimum Requirements: Graduate from a NCBON approved School of Nursing and/or Graduate from an approved School of Nursing, BSN preferred. Current North Carolina nursing license. Minimum of two years of acute care experience, Home Health experience preferred. Maintain current AHA BLS certification. Have and maintain a valid license to operate a motor vehicle in the State of North Carolina. Use of [personal car. Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served in the Home Health area. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to his/her age-specific needs and to provide the care needed as described in the department’s policies and procedures.
