The Care Coordination RN establishes and maintains an efficient, cost-effective care management process by determining the patient’s medical and financial liability, medical necessity, and by developing, implementing and monitoring individual patient plans of care. The CC/RN assists with communicating these plans to patients, families, and to the Interdisciplinary Team to ensure quality patient care throughout the healthcare continuum and compliance with all Memorial’s policies and procedures. The CC/RN is responsible for maintaining the knowledge and skill set related to utilization review, care coordination, performance improvement and professional licensure and certification.
QUALIFICATIONS AND REQUIREMENTS
Education: Bachelor’s degree in Nursing required. Master’s degree preferred.
Experience: Two years' recent acute care experience required. Two years' of Utilization Review/Case Management experience and one year experience in discharge planning in an acute care setting preferred.
Licenses/Certificates/Registration:Washington State (DOH) Registered Nurse licensure required. Case Management certification (ACM, CCM, RN-BC) preferred.
