Minimum of five (5) years’ experience in planning, development, and administration of health social services within a healthcare, environment or programs at local community, county or state level to include Minimum of two (2) years’ experience in developing and supporting, collaborative partnerships both internal and external related to the, health of communities or 2 yrs. experience in community outreach, Social, Determinants of Health (SDoH) related need identification and resource, allocation.
Ability to manage resources and coordinate across areas and cross functionally.
Demonstrates participatory style, high energy, and a bias for action, with excellent judgment.
Excellent written and verbal communication skills.
Ability to facilitate groups effectively.
Ability to analyze reports and use data to drive, performance results.
Passion for improving the lives of under privileged individuals, and families
Demonstrated experience in developing relationships with community organizations and organization leaders
Experience with case management in the clinical environment and/or the management of chronic conditions.
Experience with oversight of budget and fiscal management
Bachelor's degree in healthcare administration, public health, social work, public policy or related field OR four (4) years of experience in a directly related field.
