Providence is calling a Registered Nurse (RN) (1.0 FTE, Day shift) to work in Sound Home Health Thurston County and surrounding areas in Washington.

In this position you will:

  • Admission, recertification, and discharge assessments.
  • Create and modify care plans
  • Medication reconciliation
  • Point of care documentation
  • Assess wounds and ostomies and make recommendations to the RN case manager for optimal wound and ostomy management.
  • Collaborate with the care team and providers
  • Obtain orders for services and enter in the EMR
  • Provide information to patients & significant others regarding diagnosis, interventions & available resources.
  • Demonstrate understanding of and exhibit support for organizational behavior guidelines.

Qualifications:

Required qualifications for this position include:

  • Graduate of an accredited school of nursing.
  • 2 years nursing experience, including one year direct-patient experience with wound certification
  • Current Washington State Registered Nursing License
  • Valid Washington State driver’s license
  • Insured and reliable transportation
  • Current Wound Certification (CWOCN, WOCN, CWCN, WCC, CWS, CWCA)
  • Healthcare Provider BLS/CPR certification

Preferred qualification for this position include:

  • 1 year Home health care experience
  • Prefer experience with wound certification in an inpatient or clinic setting
