We are looking for an Intake/Admission RN for Memorial Hospice!

Schedule : Full time - 64 Biweekly hours

Job Summary :
An Intake Assessment Nurse RN is a registered nurse with home health experience whose primary responsibility is to accept referrals from a variety of resources via phone and fax with additional responsibility of co-signing referrals entered by LVN's and or non-clinical intake staff. The nurse plays an active role in both the office based intake and the field admission process, and will be face-to-face with patients who are being considered and/or admitted to Hospice services. This is a clinical RN position.
