Job Description

Key responsibilities will include developing relationships with providers & staff by working "elbow to elbow"on workflow & efficiency sessions, review and assist with patient panel & care gap closure; provide report assistance to manage patient care opportunities, documentation review, support during process change, onboardingor EMR upgrades. QI Coordinator RNs will be licensed to place health maintenance orders, possess the knowledge to assist with medication adherence, skills fairs, process improvement, training and perform RetinaVue screenings. Bachelor's degree in nursing preferred; active NC RN license required. Intermediate knowledge and proficiency in computer systems especially Windows, Microsoft Office, and Power Point. Five years' experience in the ambulatory care setting. Minimum of three years' experience working with healthcare quality and value-based programs with current knowledge of programs such as MIPS, Hedis, PCMH and MSSP preferred. Minimum of 2 years' experience with Lean and/or Six Sigma preferred. BLS required. Clinical experience in various settings including experience with quality measures & population health required.
