SUMMARY:
The Licensed
Practical Nurse provides nursing care according to the physicians’ orders and
in conformance with the ANA Standards of Practice and Care, ANA Code of Ethics,
unit specific standards, patient care policies and procedures, unit specific
policies and procedures, and the Louisiana Nurse Practice Act. The LPN will assist in maintaining a safe,
neat and clean physical enviroment in the unit.
Under the direction of the Registered Nurse, the LPN is proficient in
performing supportive patient care and reports the patient’s signs, symptoms
and any unusual occurrences.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

The
Licensed Practical Nurse works under the direction of the Registered Nurse
·
Obtains
information to contribute to an accurate and current patient assessment,
patient care and discharge plan
·
Delivers
compassionate care to patients and supports members of the patient’s family
·
Includes
the patient and caregivers in this process to obtain accurate
information/history and contributes to the RN’s assessment
·
Evaluates
the patient’s needs, reports findings in a timely manner, evaluates the patient
frequently to determine changes in condition/needs and performs basic nursing
monitoring skills, as well as contributes to a current patient
assessment-I&O, IV, VS
·
Gathers
information appropriately using sound nursing assessment skills, proper
interviewing techniques and collaborates with the multi-disciplinary team in
discharge planning
·
Demonstrates
competency and knowledge of current standards of practice
·
Accurately
performs current practice standards and performs basic nursing skills in a
competent manner
·
Administers
medication correctly and carries out orders promptly
·
Is
responsible for accurately documenting patient care in a timely manner being
attentive to detail when documenting procedures, medication, IV maintenance and
abnormal findings
·
Should
demonstrate knowledge of disease process, recognize abnormal conditions and
reports valid patient care conditions and changes
·
Provides
patient care in an organized manner, utilizing knowledge of nursing
process-assessments, planning, interventions and evaluations
·
Under
the direction of the direction of the RN, the LPN will interpret the
information needed to identify each patient’s requirements relative to the age-specific
needs
·
Provides
individualized health education to the patient and caregivers to specifically
meet the patient’s needs related to the current diagnosis
·
The
education process is from admission through discharge and includes all age
groups pertaining to the patient population served
·
Provides
care in a safe and therapeutic environment
·
LPNs
assigned to Hyperbarics Department: Properly operates the T Com machine, the
laser Doppler and CBG machine. Under the
supervision of the RN, the LPN also participates in wound care management,
assists the physician with debridements, biopsies and suture set up as well as performing
wound photography and documenting

EDUCATION/TRAINING/EXPERIENCE:

Maintains current
licensure as Licensed Practical Nurse in the State of Louisiana

CPT Certification

IV Therapy

PHYSICAL
DEMANDS AND WORKING CONDITIONS:

Medium work as defined by the US Department of Labor
constitutes a maximum lift of 21-50 pounds on occasion and/or a maximum lift of
11-25 pounds on a frequent basis
·
May be required to assist in the turning, lifting or moving
of patients as well as walking about from patient’s rooms, as well as to and
from various hospital areas
