Augusta Health's Home Health team is seeking a part time Registered Nurse - Care Manager to join their team.
The Registered Nurse-Care Manager provides skilled nursing care to patients in their homes, including IV therapy, wound care, etc. and directs and guides patient/family education and the activities of other personnel while maintaining standards of professional nursing. The ideal candidate must possess the ability to assess data reflective of the patient's status and interpret the information to formulate a plan of care based on the patient's age-specific needs. Must be able to provide the care as needed, as described in the department's policies and procedures and/or per a physician's orders.
