Job Description

Provides oversight for all telehealth activities to include clinical and fiscal responsibility. Develops and manages a high functioning and competent Complex Care Management team with a focus on patient centered and outcomes driven care. Assures the development and implementation of policies and procedures, including a method to verify staff competence and compliance. Coordinates referral and the client/patient enrollment process ensuring selection of equipment meets the requirements specific to client/patient needs. Assures staff is properly trained and competent in the use of telehealth equipment. Develops and implements efficient and effective processes for the Telehealth Center. Provides needed support to case managers and other team members. Ensures appropriate interventions are made and documentation is complete. Manages and tracks clinical performance and outcomes for patients on Telehealth. Ensures health care practices meet the professional quality assurance standards of FirstHealth and applicable regulatory bodies. Coordinates deployment of all telehealth equipment. Manages telehealth configuration and set up. Coordinates retrieval of all telehealth equipment. Resolves all technical telehealth issues. Assists in the management of other technical equipment as directed. Triages patient data. Implements disease specific best practices. Maintains Survey Readiness. Reviews all aspects of documentation and follow up to ensure timeliness, accuracy, and completeness. Compiles and analyzes Telehealth Center/Complex Care Management data as directed. Present specific reports, as needed, describing program outcomes and progress toward goals. Assist in program dissemination as directed.Communicates regularly with community partners. Participates in the performance improvement process. Current North Carolina RN license or compact state license required. Three years of current experience as a Registered Nurse with a strong medical-surgical background. Three to five years of home health experience and one year of nursing supervision or management experience required. Budgetary experience preferred. Data collection/analysis experience preferred. Experience with Remote Patient Monitoring preferred. Project/Grant Management experience preferred. Working knowledge of all regulatory guidelines and standards preferred. Evidence of current CPR certification. Valid North Carolina Driver's License and proof of autoliability insurance. ]]>
