Job Description

Position Objective:

This position is responsible for providing direct patient care in the area of tobacco cessation and cancer prevention by effective collaboration with other members of the health care team. Supports the Cancer Prevention Program and the Oncology Initiative by assisting patients become tobacco free and functions as the role of case manager. Under the direction of the Executive Director, will assist in coordinating all aspects of the Cancer Prevention Program.

Essential Job Duties:

  1. Consistently assures that all appropriate patients/clients are identified and recruited to participate in various cancer prevention/tobacco cessation programs.
  2. Consistently and thoroughly assesses patients/clients to collect data and identify learning needs according to established standards and policies
  3. Develops, individualizes and implements a plan of care with regard to tobacco cessation and cancer prevention for each identified patient in accordance with established standards, policies, procedures and guidelines. Plans of care address the physical, psychosocial, spiritual and learning needs of the patient/family.
  4. Continuously evaluates the effectiveness of the plan of care, making revisions and recommendations based on analysis of patient responses to interventions.
  5. Works with hospital staff, staff nurses, care coordinators/discharge planners and physicians, to ensure that appropriate patients are identified for tobacco cessation consultation; nicotine replacement therapy and post-discharge follow-up.
  6. Using established procedures, responsible for coordinating and conducting follow-up activities for identified patients, including, but not limited to telephone calls and mailings.
  7. According to hospital policy and program standards, will be responsible for assuring the proper collection and maintenance of all program data. Will assist with maintaining and analyzing patient databases.
  8. Provides outreach to hospital staff and the community to update progress on the Cancer prevention/Tobacco Cessation Programs.
  9. Participates in the education and orientation of new staff, as necessary. Participates in QI, CPI and risk management activities at the department or organizational level (e.g., Performance Indicators for AAMC).
  10. Performs other duties as assigned (e.g., health fairs/community events).

Educational/Experience Requirements:

  • Associate degree in Nursing.
  • 3-5 years of experience in nursing is required.
  • Must have high level of interpersonal and communication skills to handle sensitive and confidential situations.
  • Knowledge of behavior change as it relates to tobacco cessation and cancer prevention as well as nicotine replacement therapies is highly desired.

Required License/Certifications:

  • Current licensure as a registered nurse by the Maryland Board of Nursing.
  • Adherence to the credentialing requirements of AAMC as stated in the nursing bylaws.

Working Conditions, Equipment, Physical Demands:

There is a reasonable expectation that employees in this position will be exposed to blood-borne pathogens.

Physical Demands –

Medium work. Exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
