Job Description

Pikes Peak Hospice and Palliative Care is looking for an experienced Registered Nurse to work in our In Patient Unit on an as needed basis. Pikes Peak Hospice & Palliative Care has been the community leader in hospice and palliative care for 38 years, founding the hospice movement in the Pikes Peak region in 1980. We are the only nonprofit, community-based hospice provider in El Paso County. Clinical employees will participate in a team oriented environment, assisting in the coordination of care to meet the needs of our patients/families in home care, long term care and assisted living facilities, and our own licensed inpatient care unit, ensuring that quality care is provided to all.

POSITION SUMMARY:

Provides patient care using nursing skills and processes in accordance with applicable scope and standards of practice, keeping within the policies, values and mission of the organization. The RN is a member of the Interdisciplinary Team/Group (IDT/IDG).

KEY REPORTING RELATIONSHIPS:

  • Reports to the Clinical Manager or Director of Clinical Services.

ESSENTIAL DUTIES:

  • Provides skilled nursing coordination of care to patients (neonates to geriatrics) as determined by the Plan of Care (POC). Uses education, mentoring, guidance and interventions to achieve goals of care.
  • Collaborates across disciplines to coordinate patient care; participates in the POC in IDT/IDG meetings as assigned.
  • Demonstrates appropriate nursing care, including communicating with attending provider for medications and supplies.
  • Educates patients, families, caregivers and community regarding hospice philosophy and palliative treatments, if applicable.
  • Supervises Certified Nurse Aides (CNA); delegates specific tasks per agency policy.
  • Participates in related services, such as community education, team training, staff/board committees and orientation of employees.
  • Other duties and responsibilities as assigned.

If assigned to Inpatient Unit/critical care unit:

  • Meets all the essential duties assigned to RNs.
  • Understands the acute care needs of patients admitted to the Inpatient/Critical Care Units.
  • Completes initial nursing assessment and admissions processes for newly admitted patients.
  • Manages customer service needs that are unique to a care unit.

if assigned to outpatient:

  • Meets all the essential duties assigned to RNs
  • Adapts care to meet the needs of patients at their place of residence including, but not limited to, the patients’ home, long-term care or assisted living facilities.
  • Completes initial nursing assessment and provides ongoing comprehensive nursing care through medication, treatment, intervention and evaluation.
  • Develops and maintains professional business relationships with facilities by providing excellent customer service to the patient, as well as the facility staff.

If assigned to Triage:

  • Meets all the essential duties assigned to RNs.
  • Assesses symptom care needs by utilizing critical thinking and organizational processes. Educates patients and/or caregivers; deploys clinical staff to make patient visits.
  • Uses professional and customer service-based demeanor while communicating by telephone, text or email.

CLINICAL CORE COMPETENCIES:

  • Supports the mission and values of hospice, palliative and home health care.
  • Provides patient care (neonates to geriatrics) as determined by the Plan of Care (POC) and in accordance with discipline. Utilizes critical thinking skills, education, mentoring, guidance and interventions to achieve goals of care.
  • Possesses knowledge of patient rights; ensuring an atmosphere which allows for the privacy, dignity and well-being of all patients in a safe and secure environment.
  • Understands the correlation between excellent patient care and the results of Hospice Item Set, CAHPS and other publicly-reported industry and/or accreditation quality benchmark programs.
  • Treats patients, family members and other clients with concern and empathy; respects their confidentiality and communicates in a courteous and respectful manner. Maintains and respects professional and personal boundaries. Adheres to HIPAA regulations, and maintains confidentiality of all patients and families.
  • Performs job duties within the standards of practice framework.
  • Has proficient nursing skills and pain management knowledge, as well as principles of pharmacology including, medication dosages, calculations, interactions and side effects.
  • Works independently with minimum direction, anticipates and organizes workflow, prioritizes and follows through on responsibilities.
  • Attends team meetings and in-services on a regular basis. Participates in continuing education and professional development courses, to include annual education requirements as assigned to the discipline.
  • Works in a high volume environment with rapidly changing priorities.
  • Understands the importance of providing accurate and timely point-of-care documentation in Electronic Medical Record (EMR). Uses, clear concise and professional written and verbal communication.
  • Responsible for meeting visit frequency, case load and/or visits per shift requirements; takes on-call rotation as needed and/or assigned.
  • Possesses knowledge of necessary computer systems including Microsoft Office, Outlook and agency electronic medical records (EMR).
  • Promotes a holistic approach to patient care.
  • Cooperates with all co-workers; demonstrates flexibility to perform other duties as assigned.

POSITION QUALIFICATIONS:

  • Minimum Education: Graduate of an accredited school of nursing; BSN preferred.
  • Minimum Experience: One to two (1-2) years of experience in hospice and/or palliative care, medical/surgical, public health, oncology, acute care or home health.
  • Required License: Current Colorado RN license. Valid Colorado state driver’s license and proof of auto insurance.
  • CPR certification (CVNA, Pathways, The Denver Hospice and Pikes Peak affiliates).

PHYSICAL REQUIREMENTS:

  • Ability to lift/push/pull minimum of 50 lbs.
