Job Description

ROLE SUMMARY

Works under the direct supervision of a registered nurse or other professional, offers compassionate care to all patients based on age-specific needs/expected outcomes, placing an emphasis on high quality, comprehensive treatment, by means of an interdisciplinary team and using a holistic approach. Provides personal care, homemaking, emotional, social and spiritual support to patients and families – regardless of race, age, sex, religion, handicap or ability to pay during all phases of illness and bereavement.

CORE RESPONSIBILITIES

  1. Assists patient with personal care. Assists patient with transferring and ambulation. Assists patient with prescribed exercises and simple procedures which have been taught by appropriate professional personnel.
  2. Performs homemaking services which are essential to the patient’s care at home.
  3. Assists in maintaining a safe environment for the patient. Use safety and patient care equipment appropriately. Reinforce the safe use of equipment to the patient.
  4. Uses proper body mechanics when providing patient care/homemaking services. Use universal precautions in providing services. Recognize limitations and seek assistance as needed.
  5. Observes patient condition and immediately communicate changes/concerns.
  6. Records care given and pertinent changes in the patient’s condition on the Aide Flow Sheet.

ROLE SPECIFIC RESPONSIBILITIES

HOME CARE SCHEDULER: Schedules initial and continuous visits for intermittent and hourly programs across various disciplines. Abides by CMS rules and regulations to schedule supervisory visits for appropriate disciplines. Works closely with Intake RN and Billing specialist to ensure that visits are appropriately scheduled in accordance with payor preauthorization, CMS rules and provider orders.

SKILLS, KNOWLEDGE, AND ABILITIES

  • Ability to interact positively with patients, patient’s families and staff so as to promote positive patient care
  • Ability to be flexible with department needs. Communicates pertinent information to appropriate persons
  • Ability to demonstrate sufficient dexterity to provide patient care. Ability to speak clearly for patient communication
  • Ability to operate motor vehicle is necessary
  • Must have available reliable transportation to and from assignments
  • Must provide proof of insurance for personal vehicle used for transportation

MINIMUM REQUIREMENTS

  • High school diploma or GED
  • Minimum of a current Wyoming Certified Nursing Assistant license upon start date
  • AHA BLS
  • Valid USA driver’s license
  • Must have clean driving record that follows Cheyenne Regional Medical Center’s driving guidelines

REQUIREMENTS AFTER START

Additional home health / hospice training within 30 days of start date
