Position Summary

  • The nurse navigator provides individualized assistance to specific patients and their families in overcoming barriers to care, facilitating timely access to care, and providing education and support.
  • The patients' trajectory through diagnosis, treatment, and after care will be seamless and coordinated in collaboration with the multidisciplinary team.
  • Additional patient navigational needs may be assigned to this Navigator.

Minimum Requirements

  • 4 year college degree is required.
  • Current RN North Carolina nursing license is required.
  • Excellent communication skills, both written and oral.
  • Organized, can multi-task, analytical, time management.
  • Work well under stress and be able to provide emotional support.
  • 3-5 years clinical experience for patient focus is required.
  • 3-5 years in a hospital setting is required.

Preferred Requirements

  • Certification for patient focus preferred or within 2 years of employment.
