RN Case Manager

Role Summary:

The RN Case Manager assists with coordination of services through the continuum of care and in balancing quality patient care with cost-effective utilization of resources. The RN Case Manager role is responsible for clinical reviews, discharge planning, Post-acute or discharge agency referrals and care coordination.


Department/Unit Summary:

Case Management: Encompasses Case Management, Social Work and Utilization Management roles. The Case Manager role averages 15-20 cases. Case Managers work 8-hour shifts; M-F between 6:30am-10:30pm; with one weekend a month rotation. Emergency Case Managers work 3-12 hr. shifts or 4-10 hr. shifts; every other weekend.

The two-campus facility is the sole provider of inpatient and outpatient medical services in Wilson County.
