The VA Care Manager-RN 2 is responsible for the management of patient care in all the assigned LLUHC/VA Community Based Outpatient Clinics (CBOC) PACT Teams. This includes coordination of assigned high risk patients in order to expedite patient movement throughout the health care setting and facilitating continuity of care. Evaluates daily physician/provider assigned patients in the CBOC’s, those transitioning from inpatient to ambulatory care status and those receiving home care support, collaborating with the Home Health Service, Care Managers, and Social Work. Serves as a consultant and resource person to other members of the health care team. Works closely with CBOC Clinic Manager, LLUHC/CBOC Medical Director, LLUHC/CBOC Director, and VA Coordinator as needed. Performs other duties as needed.

Associate's Degree in Nursing required. Bachelor’s Degree (BSN) preferred. Minimum two years as a Registered Nurse in a clinical setting, with patient triage and assessment experience required.

Able to read; write legibly; speak in English (and Spanish preferred) with professional quality; use computer and software programs necessary to the position; troubleshoot and calibrate patient care equipment. Able to relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following and/or enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate. Able to accept direction; perform basic math functions; manage multiple assignments effectively; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail. Able to distinguish colors and smells as necessary for patient care; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place/patient care; see adequately to read computer screens, medical records, and written documents necessary to position; discern temperature variances through touch.

Current California RN License unrestricted required. CPR (BLS) Certification by American Heart Association required. Valid driver’s license and auto liability insurance required.
