This position will be a telephonic role providing reliable, high-quality service that allows patient referrals and transfers to be accepted at our SCL Health facilities in the Denver metro, Colorado region. The work location will be at our Saint Joseph Hospital campus in downtown Denver.

Your day.

As a Transfer Center RN you need to know how to:

  • Provide the assessment, planning, facilitation, and support for optimal patient transfers into SCL Health facilities in the Denver metro, Colorado region.
  • Use clinical knowledge and critical thinking and communication skills to ensure the transfer center provides a reliable, high-quality service that allows referrals and transfers to be accepted in an efficient and timely manner.
  • Assess, direct and coordinate transfers from physician offices, hospitals, and other healthcare setting to the most appropriate accepting service, facility, and level of care.
  • Assist with registration, transportation and bed arrangements.
  • Demonstrate expertise and resourcefulness in level of care requirements, bed capacity management, service line capabilities, and acceptance pathways by care site.
  • Adhere to EMTALA regulatory guidelines for patient transfers.
  • Provide timely and accurate transfer documentation.
  • Work collaboratively with our providers and key stake holders in the development and maintenance of transfer and admission pathways for our patients.
  • Delegate as needed to the Transfer Center Technician.

Your experience.

We hire people, not resumes. But we also expect excellence, which is why we require:

  • Bachelor’s Degree in Nursing (BSN)
  • Current Colorado RN license or compact license with the ability to obtain Colorado RN license
  • Current BLS certification endorsed by the American Heart Association
  • Minimum of three (3) years of experience in acute care nursing

Preferred:

  • Current ACLS certification or other specialty certification (CCRN, PCCN, MSRN, or CEN)
  • Critical Care or ED background, Hospital Supervisor or Charge Nurse experience, experience with hospital throughput or capacity management
